Incidents, Injuries and Concerns

The University of Wisconsin-Madison Environment, Health & Safety (EH&S) Department is dedicated to ensuring a safe and accessible campus for all who work on and travel to campus. EH&S uses various tools and collaborates with campus stakeholders to monitor workplace safety and assist in identifying, assessing, and mitigating risks or uncertainties that can affect university goals, initiatives, and objectives.

To maintain campus safety and accessibility, employees are encouraged to submit the Near Miss – Unsafe Condition Report and/or the Employee Work Injury and Illness Report. Please select each button to learn more.
